• Enter all information requested by State Department (red asterisk fields are required)
• Once you get to "Summary of Fees" page (Check $60 Expedite Fee) and click "SUBMIT"
• Next Page - Scroll to bottom and click "CREATE FORM"
• Your Passport Application form will open as a PDF document, PRINT IT OUT

• New Passports: Is this what you need? Ask yourself these questions:
• Am I applying for a passport for the first time?
  Was my prior passport issued at age 15 or under?
  Was my prior passport issued more than 15 years ago?
  Is my expired passport not in the government's possession?
  Is my current, valid passport damaged
