• Enter all information requested by State Department (red asterisk fields are required)
• Once you get to "Summary of Fees" page (Check $60 Expedite Fee) and click "SUBMIT"
• Next Page - Scroll to bottom and click "CREATE FORM"
• Your Passport Application form will open as a PDF document, PRINT IT OUT

• Passport Renewals: Is this what you need? Ask yourself these questions:
• Was my prior passport issued when I was age 16 or older, (And)
  ....the passport is still in my possession and undamaged, (And)
  ....the passport was issued less than 15 years ago?
